How do I make an appointment?

How soon will I receive my appointment?
We will do our best to work around your schedule and make an appointment that is convenient for you. Crossing Healthcare hours are Monday and Tuesday 8:00AM-7:00PM, Wednesday, Thursday and Friday 8:00AM – 5:00PM. We offer walk-in visits Monday-Friday 8:00AM-4:30PM. For more information, click here.
I have an appointment scheduled. What do I need to bring on the day of my visit?
On the day of your appointment, please arrive 15 minutes early to register. Please bring the following information that is required to complete your patient registration:
- A Photo ID such as:
- Driver’s license
- State identification
- Valid passport
- Veteran’s ID
- Employment or school ID
- Another form of identification such as:
- Social Security card
- Alien registration card/”green card”
- Current utility bill
- Your current address and phone number
- Medicaid, Medicare or insurance card (if you have one)
- If you don’t have a Medicaid, Medicare, or insurance card, please bring income information, such as:
- Two paycheck stubs
- W-2 form
- Illinois Link Card
- Unemployment notice

I need after-hours care. What do I do?
If you have a medical emergency, please call 911 or go to your nearest emergency room. If you have a health concern that is not an emergency after regular hours, our phone system switches to an answering service. One of our on-call medical providers will assist you with your health concern. For all other business, including test results, scheduling an appointment and medication refills, please call during our regular business hours.
How do I cancel an appointment?
If you are unable to keep your appointment, please call us at 217-877-9117 as soon as possible so we can reschedule your visit and offer your time slot to another patient.
Do you accept health insurance?
Yes, we accept most insurance plans. You should check with your insurance provider to ensure we are one of your approved providers. We also accept Medicaid and Medicare. Additionally, all pregnant women who do not have insurance are eligible for Illinois Medicaid programs. Our staff can assist you in completing Illinois Medicaid applications.
I don’t have health insurance. Can I still become a patient?
Absolutely! We are committed to providing quality healthcare that is accessible to everyone, regardless of insurance status. If you don’t have insurance, we offer programs to help. While our locations aren’t classified as free clinics, we strive to make our services affordable. We accept cash, checks, and credit cards, and we can work with you on a flexible payment plan that fits your needs.
Patients who do not have insurance may qualify for our sliding fee program, which is based on your household income and family size. Depending on your situation, you may save 25 percent to 100 percent off our regular fees.
