Care Management Services
Crossing Healthcare offers care management services to help patients prevent and manage their medical conditions, such as prediabetes, diabetes, high blood pressure, and high cholesterol.
Crossing Healthcare’s Care Managers work in partnership with the medical team to provide additional support to patients with chronic conditions. Care Managers provide patients with resource support and education, including assistance with a better understanding of medications. Care Managers may refer patients to community and social service resources for additional help and support if needed.
