Crossing Healthcare and Decatur Public Schools are partnering to provide healthcare services to Decatur Public Schools Students through a mobile unit. This partnership will allow Crossing Healthcare to provide convenient access to quality, integrated, healthcare.
The mobile unit will be accessible to Decatur Public Schools students at the start of the 2019-2020 school year. Services offered on the mobile unit will range from physicals, vaccinations, health screenings, and treatment of minor illnesses and injuries, to health education and promotion, nutrition counseling, and behavioral health counseling. Dental services will be offered, starting in April 2020.

To have your child recieve healthcare services on the mobile unit a patient registration, health history, and consent form must be completed. Each of these forms can be downloaded below. Completed forms can be returned to Crossing Healthcare by faxing them to 217-877-3079, mailing or dropping off to Crossing Healthcare, 320 East Central Avenue, Decatur, IL 62521, or by returning to your child’s school nurse.
